Welcome to Child Evangelism Fellowship of South Santa Barbara County's FREE 5-Day LEGO Club! We are happy and excited to offer our FREE Lego Camp to the children and their families again this summer. Over the years it has been our desire to reach the children in our community who have never heard the Gospel of Jesus Christ. CEF Santa Barbara will open the Lego Club to these families first, with the hope that many families will want their children to hear the Gospel. If you have family friends that would want their children to learn about Jesus, while having a blast, please invite them to register. CEF asks that you add your children to our waiting list. HOWEVER, if your child has an unchurched friend please invite them to register (separately) and you can register your child (include the name of your friend in comments). Please partner with us as we reach the unchurched. Thank you for understanding and your faithful support and prayers over the years.

WHO: Elementary school children: MUST have completed KINDERGARTEN

5-DAY CLUB RELEASE AND CONSENT
Note: this form takes the place of the printed Release and Consent form. No need to email a separate form as in previous years. Thank you.
Liability Release and Consent for Treatment: In the unlikely event that my child is injured while participating in activities at 5-Day Club® and other official CEF activities on or in route to such activities, my child and I relinquish all rights to recover damages for any or all injuries sustained to my child. In consideration for CEF granting my child permission to participate in CEF activities, I hereby release Child Evangelism Fellowship of Southern California, its employees and volunteers from liability or injuries occurring in CEF activities.
In case of emergency, I hereby authorize CEF South Santa Barbara County Chapter to contact emergency personnel and release pertinent personal information so that my child may receive treatment.
Liability Release, Consent for release of information and for treatment.
Model Release (Use of Photographs): Pictures or video may be taken during the 5-Day Club for use of promoting CEF activities. I ALLOW CEF to use photos of my child for promotional and advertising purposes.
TRANSPORTATION: Please be aware there will be no transportation provided by CEF for your child/ward to return home, and you therefore need to make arrangements for them to be picked up at 12:00 p.m. each day.
